How to request speech, language and communication support

Making a request for service / referral

Anyone can make a request for speech, language and communication services with consent from the person with parental responsibility for the child or young person.

All children under 11 years old should have a WellComm screening score included in the request for services. More information about WellComm can be found below.

The information within the request for service form provides a starting point for our team.

Form submission trouble shooting suggestions:

  • The date of birth will not turn blue, is this ok? Yes, this is as expected.
  • If there are any other lines that remain pink, there is data missing and this needs to be complete before the form will submit. 
  • If you have copied and pasted any data into cells asking for an email address or contact number, please check there are no blank spaces after the text as again this will top the form from submitting.
  • Please use the free text boxes to add any information you feel is relevant to us. 

What happens after a referral is submitted?

The referrer will receive an automated email to confirm that the form has successfully been submitted. Please check your junk mail folder as this acknowledgement may be there.

Following a request:

  1. For all referrals received post 01/02/2025, the referrer and each parent/carer detailed in the referral form will receive a formal acknowledgement of the referral by email.  We aim to send this within 3 weeks of referral to us. That acknowledgement will also include links to information and guidance about supporting children and young people’s speech, language and communication development.
  2. If parents/carers change your telephone number, email address or move home, please email slt.halton@communicate-slt.org.uk to let us know.
